The School of Continuing Studies is committed to providing an inclusive and positive learning experience for every learner who chooses to take courses with us. This form is required to ensure that the best supports are implemented to meet each learner’s individual needs, promoting a positive and barrier free learning environment for all.

Protection of Privacy

In accordance with the Freedom of Information and Protection of Privacy Act (FIPPA), the information shared on this form and the supporting documentation provided is collected for the purpose of providing appropriate and reasonable accommodations and supports for identified learners with a disability. Personal health information is kept confidential within the School of Continuing Studies Accessibility Team and is not shared with any external individuals, including Instructors, unless the learner provides written consent to do so. Steps to Register

  • Step 1 — Complete and submit this form.

    There is no deadline to register for accommodation, nor do you have to be enrolled in a course to request or plan accommodations with our office. Early registration is recommended so our Accessibility Team can do their absolute best to implement the requested supports by the start date of any upcoming courses.

  • Step 2 — Schedule an Appointment

    This is not a formal consultation. This is your opportunity to meet with a member of the Accessibility Team to discuss the information provided on this request form and for YOU to ask any questions or concerns regarding upcoming courses. This appointment can be completed by phone, in-person, or by email

  • Step 3 — Keep Us Informed

    Let the Accessibility Team know if you need to make any changes in your course registration (including withdrawals and/or transfers) or any upcoming items (assignments, tests, etc.) for which you may require support.

Supporting Documentation

Before our office can successfully implement any accommodation requests you identified above, documentation from a medical professional or educational institution must be provided.

We ask for documentation within the last 5 years to ensure that the recommendation best reflects current needs. If more that 5 years, or if any item on the report requires clarification, we may ask for additional supporting documentation.

You do not have to be enrolled in order to request accommodation. In fact, we encourage you to place your request well in advance of registration to ensure that we can accommodate you in a timely manner.

Note: If you are requesting accommodations, but do not yet have the necessary documentation to proceed with this request, interim accommodations may be arranged. Important Deadlines

The deadlines indicated below are not firm, however, the sooner our Accessibility Team is made aware of your request the greater opportunity we have in successfully making these arrangements for you as needed. We may not be able to accommodate if we are not notified by the deadlines stated below.

The deadlines to notify us of your needs are as follows:

Alternate Format Requests 2-4 weeks prior to the course start date to allow time to obtain an accessible format prior to the start date of any course.
Note-taking Requests Within 1 week of the course start date
Tests/Exam Requests Within 2 weeks of any test date you require support for. (Not including final exams)
Final Exams (online courses) Submit your Exam Application form as outlined in your course, and then immediately notify our Accessibility Office of the supports required for this exam. Our team can work with the Exam Office to make arrangements for you as needed.
Final Exams (In-class courses) No later than 6 weeks prior to the final examination date.
